Sorry for the poor quality of the picture.  I couldn't figure out how to copy the picture to upload here, so after several attempts, I finally just copied into word, saved as a pdf and then through photoshop, turned it into a jpeg file.  Sheesh!  If I didn't have photoshop, y'all would have just to have taken my word for it!
